Industrial accidents on building sites are rare in Sweden compared to other countries.
Most likely it was a mapping error. The good thing is that in Sweden the maps will now be updated to correct the error.
Making outdated maps the scapegoat when losses of life could occur from the negligence of the drilling company to properly follow through with all safety procedures before beginning to drill will cure nothing.

Do you even realize how hard it is to check if there is a subway tunnel 25 meters under where you are drilling, without actually drilling? People these days are outraged when planes crash, while they should be saying: "I am FLYING!" Same here; without accurate maps these things just happen, and either the drilling company made an aiming error (very gross negligence!), or the public subway maps are wrong, and should be corrected and rechecked as quickly as possible.
